By Dr. Jaray Krainam, MD

Tip Plasty and Tip Graft Surgery are refined cosmetic nose procedures designed to reshape and enhance the nasal tip. These techniques are ideal for patients who have a bulbous, wide, square, droopy, upturned, or poorly defined nasal tip, or those who simply want a more elegant and balanced nasal profile.

As one of Thailand’s most experienced rhinoplasty surgeons with over 20 years of specialized expertise, Dr. Jaray Krainam, MD uses advanced cartilage manipulation and grafting techniques to create a natural, structured, and beautifully refined nasal tip that suits each patient’s facial proportions.

How Tip Plasty & Tip Graft Work

The procedure may include:

  • Reshaping or reducing the alar cartilage
  • Strategic suturing to refine tip definition
  • Removal of excess fibroareolar or subcutaneous tissue
  • Tip grafting for improved structure and projection
  • Columellar strut graft for support and stability
  • Elevation and refinement of the nasal tip for better contour

These advanced methods allow precise reshaping of the nasal tip while maintaining long-term stability and natural expression.

Operation Time

Approximately 1 hour, depending on the complexity and the amount of reshaping required.

Care After Surgery

  • Mild discomfort, pressure, or tenderness is expected for the first few days.
  • Swelling and light bruising usually diminish within one week.
  • Pain can be managed with prescribed medications.

Recovery Period

  • Most patients recover within 3 weeks.
  • Sutures are typically removed within 10 days.
  • Light activity can resume early, but strenuous exercise should be avoided until cleared by the surgeon.

Expected Results

Dr. Jaray provides highly refined, natural-looking, and structurally stable tip results.
If correction is required due to surgical factors, a no-charge revision is provided.

Risks and Complications

While complications are uncommon, potential issues include:

  • Asymmetry
  • Graft movement
  • Cartilage weakening or collapse (with inexperienced surgeons)

Because the nasal tip is structurally delicate, tip plasty is considered one of the most challenging rhinoplasty procedures. Choosing an experienced surgeon is essential to avoid tip collapse, poor projection, or long-term deformity.

Type of Anesthesia

Performed under:

  • Local anesthesia, OR
  • General anesthesia, depending on complexity and patient preference.

Hospital Admission

Patients are discharged on the same day of surgery.

Preparation Prior to Surgery

  • Complete medical evaluation (history, medications, allergies, lab tests)
  • Stop smoking 3–4 weeks before surgery
  • Avoid alcohol 1 week before surgery and during recovery
  • Avoid hormone medications, anticoagulants, steroids, and supplements for 4–6 weeks
  • Avoid aspirin, anti-inflammatory drugs, and herbal supplements that increase bleeding risk
